| Drivers Medical for Seniors (75 and above) | $136.50 |
| Forms (Disability tax credit, AISH, Revenue Canada and others) | $180 and upwards depending on complexity |
| School forms/ Camp forms/ Dayhome forms/ etc | $52.50 |
| Alberta Blue Cross Special Authorization form | $52.50 |
